Tax Form 1095-C coming

Each year, Indiana University issues Tax Form 1095-C to certain employees, as required by the Affordable Care Act. This document reports information about the medical insurance offered to employees and dependents by the university.

1095-C statements for the 2021 tax year will be distributed no later than the IRS deadline, March 2, 2022.

Keep the following in mind:

  • You do not need to wait for this document to file your 2021 federal tax return. Form 1095-C does not need to be filled out or submitted to the IRS; it is simply a reference document that should be kept with your tax records.
  • If you are signed up for electronic delivery of Form 1095-C, you will receive an email from IU Human Resources when your statement is available.
  • If you are not signed up for electronic delivery, your statement will be mailed to your home address on file. To access your statement sooner, you can sign up for electronic delivery by visiting the 1095-C Electronic Consent task in One.IU.
