Sign Tobacco-free Affidavit during Open Enrollment

The annual benefits Open Enrollment period for full-time academic and staff employees is open, which means it’s time to sign a new Tobacco-free Affidavit for 2022.
Each year during Open Enrollment, employees enrolled in the Anthem PPO HDHP, IU Health HDHP or Anthem PPO $500 Deductible Plan can sign the Tobacco-free Affidavit to reduce their medical premiums in the following year. The resulting premium reduction is $25 per month for the employee or their covered spouse, or $50 for both.
To sign the affidavit, employees can log in to the Employee Center and follow the instructions for online Open Enrollment (specifically step 5). The affidavit is located at the bottom of the Medical enrollment page and is visible once a medical plan has been selected.
All employees who want to receive the premium reduction in 2022 must sign a new affidavit during Open Enrollment, even those who are not changing their medical plans.
If you’ve already submitted your elections, you can verify your Tobacco-free Affidavit status by reviewing your Benefits Statement. After logging in, expand the “Election Summary” section and look directly above the Benefit Plan table for your Tobacco-free Affidavit status and monthly premium discount.
If you did not submit your Tobacco-free Affidavit, you can revisit the online Open Enrollment system, complete the affidavit and re-submit your elections. As a reminder, elections can be submitted as many times as needed during the Open Enrollment period.
As an alternative to signing the affidavit, employees and their covered spouses can complete the Quit for Life tobacco cessation program and receive the premium reduction.
